I understand that I am solely responsible for any harm or damage caused by my dog at Exclusive Pet Grooming (EPG). I further agree to indemnify EPG, it’s owners, staff and volunteers for any loss, liability, damage, or costs they may incur due to any harm caused by my pet(s).

 

I understand that grooming can expose a hidden medical problem or aggravate a current one. This can happen during or after grooming. I agree that any medical expenses that arise from these issues are my responsibility.

 

I understand that while all precautions are taken to keep my pet safe and comfortable at EPG, there is always a risk when grooming animals. Problems can occasionally arise from the grooming process such as nicks, scratches, quicked nails, clipper irritation, hematomas from matting, and mental or physical stress. EPG will always notify me if there is any incident with my pet during the groom, but I am responsible for any and all expenses involved. 

 

I agree and understand that any problems that develop with my pet(s) will be treated as deemed best by EPG, in their sole discretion, and that I assume full responsibility for any and all expenses involved. I authorize EPG to contact DePorre in the event of an emergency to provide appropriate medical treatment to my pet(s) if I am unable to be contacted. I agree and understand that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted by the law of the State of Michigan and if any portion thereof is held invalid, it is agreed and understood that the balance shall continue in full legal force and effect. Should a court determine that any provision waiving liability is deemed unenforceable, EPG's liability shall be limited to the funds - paid.

 

I understand that pictures or videos might be taken of my dog while getting groomed for marketing purposes.

 

I understand that my pet is required to be up to date on their vaccinations in order to receive grooming services. If I show up without proof of up to date Rabies vaccination and my veterinarian is unreachable. I will be turned away and charged the price of the groom.

 

I understand that if fleas are found on my pet, they will be given a flea bath and a $15 fee will be added to my service balance.

 

I understand that EPG has the right to refuse any services at any time. In the event that your pet is too stressed or becomes dangerous to groom, EPG has the right to refuse grooming services, stop grooming services, or cancel grooming services at any time before, during, or after grooming and client will be charged a grooming fee (for what was done up until that point).

 

I understand that EPG requires at least a 24-hour notice for all cancellations or reschedules to allow us time to fill the space with a customer from our waiting list.  If you do not cancel with in this time period you will be charged the full price of the groom.

 

I understand that EPG will hold grooming appointments for 15 minutes past the scheduled drop off time. If I show up late my dog might not be able to be groomed and I will be charged the full grooming fee.
